Service Details
| Week 1 | Receive one 8-hour visit and two additional 8-hour visits within 24 hours of discharge. |
| Week 2 | Two 4-hour visits and one 8-hour visit |
| Week 3 | Two 4-hour visits |
| Week 4 | Two 4-hour visits |
Suggested Service:
